Eligibility
1. Participants with 55 years or older with Cognitive Impairment
Client Eligibility:
Individuals with mild to moderate cognitive decline (including early-stage dementia or memory loss) who can participate in structured group activities.
Must be able to maintain personal safety with supervision (not requiring one-on-one care).
Independent in toileting, transfers, and medication self-management.
Benefit from social stimulation, structure, and safe engagement.
Ineligible:
Severe cognitive impairment with inability to follow simple directions.
Wandering behaviors that cannot be safely redirected.
Aggressive, combative, or disruptive behaviors.
Complete dependence on staff for all ADLs.
2. Participants with 55 years or older with Depressive Disorders
Client Eligibility:
Individuals diagnosed with mild to moderate depression who may benefit from structured routine, socialization, and engagement.
Must be independent in ADLs.
Capable of safe participation in a group setting.
Ineligible:
Severe depression requiring psychiatric intervention or intensive therapy.
Suicidal ideation, self-harm tendencies, or active crisis.
Individuals requiring behavioral management beyond the scope of a social model.
Supportive Measures
For Participants with Cognitive Impairment:
Structured cognitive activities (e.g., memory games, art, music).
Safe environment with supervision and redirection as needed.
Gentle orientation and reminders.
Social engagement to reduce isolation and slow decline.
For Participants with Depression:
Social and group-based activities to reduce loneliness.
Consistent routine to promote stability.
Opportunities for creative and physical engagement.
Staff trained to monitor mood and communicate changes to families.
Cognitive and physical assessment conducted by a licensed clinician is required for eligibility verification to ensure the respite modality fit the clients needs.
We do not provide clinical treatment, direct medication administration, or assistance with activities of daily living (ADLs) such as toileting, transfers, or personal hygiene.
Collaboration and Referrals:
Inspired Adult Care Center collaborates with licensed professionals, including LCSWs and other healthcare providers, to:
Assist in assessing cognitive levels, decline, and psychosocial needs.
Identify when clients require a higher level of care.
Provide families with referral support and resources.
Referral partnerships include:
Outpatient Mental Health Services for therapy and psychiatric care.
Medical Home Health Services for in-home nursing or therapy needs.
Assisted Living Facilities for clients who require more daily support.
Skilled Nursing Facilities for those with complex medical or behavioral needs.
