Making Adult Day Services Accessible:
At Inspired Adult Care Center, we understand that full-time adult day services can be a significant investment for families. To help make our programs more accessible, we offer an income-based sliding scale for eligible full-time participants. Our goal is to ensure that older adults—especially those living with dementia, depression, or other chronic conditions—can continue to benefit from high-quality day services
Required Documentation for Admission & Sliding Scale Eligibility
To complete the admission process and confirm identity and determine eligibility for the income-based sliding scale rate at Inspired Adult Care Services, PLLC, the following documents are required :
Proof of Identity: 2 forms of valid Government/State-issued ID: (Driver’s License, State ID, Medicaid or Medicare Card)
Social Security Card
Proof of medical Insurance
Long-Term Care Policy Information
Sliding Scale Eligibility (Income Verification):
Guardian's/POA: Three (3) most recent pay stubs
Guardian's/POA: Most recent W-2 form
OR
Client's: Social Security Award Letter
All documents must be current and submitted prior to enrollment. Incomplete documentation may delay admission or result in assignment to the standard private pay rate.
Accepted Payor Source: Long-Term Care Policies, VA/TRI-CARE

*{Enrollments Starting on or after 10/01/2025 Requires a Two (2) day a week enrollment requirement}
UPDATE
**Notice of Enrollment Hours**
New client enrollment is available **Monday through Friday, from 10:00 AM – 12:00 PM and 2:00 PM – 4:00 PM**.
Out of respect for current clients and existing work obligations, no enrollments will be scheduled outside of these hours.
**Licensed Practitioner Access: Contracted or partnered healthcare providers, including family-selected practitioners, are welcome to conduct therapy or assessments on-site.
